Individuals in need of rehabilitation services receive interdisciplinary care specifically designed for their particular needs. When the time is right, rehabilitation services transition to restorative services as we test the durability of newly acquired skills and help residents continue to improve their stamina. We strive to involve family and at-home caretakers to prepare them to support the resident after discharge. If necessary, we can also make in-home visits prior to discharge to identify and remediate any barriers within the environment that might present challenges to the individual’s ability to fully function independently within the home setting. |
